I have read a copy of Dance Concepts, LLC policies on the website www.danceconceptsnh.com and will take the responsibility to carefully follow the rules and policies therein. I understand that Dance Concepts, LLC does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child's participation with Dance Concepts LLC, and I voluntarily assume the risks associated with such participation.
I, the undersigned, hereby grant Dance Concepts, its representatives, and employees the right to take photographs and/or video recordings of my child(ren) during dance classes, performances, rehearsals, or any other studio-related activities. I understand that these images and/or videos may be used for the following purposes: • Promotional materials including, but not limited to, flyers, brochures, and posters. • Social media platforms including, but not limited to, Facebook and Instagram. • The Dance Concepts website. • Dance recital DVDs, photo galleries, or other digital products produced by the studio. • Any other form of media related to promoting the dance studio or documenting studio events. I hereby release and hold harmless Dance Concepts from any reasonable expectation of privacy or confidentiality associated with the images specified above. I acknowledge that the images and/or videos may be used without further notification or compensation and that all rights to the images belong to Dance Concepts. If at any time I wish to revoke this consent, I must do so in writing to Dance Concepts with the understanding that any materials already produced and distributed may continue to be used without further consent. By signing below, I acknowledge that I have read and fully understand the terms of this photo/video release waiver.
